Terms of Use
Website use, patient communications, and your rights.
Last updated: November 4, 2026.
Using this website
Soriano Physician Services PLLC dba Soriano Med provides this website for general practice information, patient resources, and links to approved services. Please use it lawfully and respect the privacy and rights of others. These terms concern website use; they do not replace signed treatment, financial, telehealth, or other clinical agreements.
Medical information and emergencies
Website articles, service descriptions, and review summaries are educational and are not personal medical advice, diagnosis, or treatment. Do not delay medical evaluation because of website content. Viewing this site, sending an administrative email, or requesting an appointment does not by itself create a physician-patient relationship. Clinical care requires appropriate assessment and acceptance by the practice.
For a medical emergency, call 911. Website email, SMS, and routine portal messages are not emergency services and are not monitored continuously. For time-sensitive concerns, call the office and follow the instructions provided.
Scheduling, care, and costs
Appointment availability, services, clinician participation, insurance networks, and eligibility must be confirmed. A booking request is subject to the scheduling platform and practice confirmation. Insurance information on this website is not a guarantee of payment. Ask your plan and the practice about benefits, coverage, and anticipated patient responsibility.
Clinical services are billed under applicable agreements and law. A portal exchange or telephone interaction may constitute a billable service when appropriate; any required consent and financial disclosure must occur through the approved care workflow. Website use alone is not consent to a billable service.
Your right to a Good Faith Estimate
If you do not have health insurance or choose not to use it, you generally have the right to a written Good Faith Estimate of expected charges for scheduled non-emergency care. You may request an estimate before scheduling. When care is scheduled at least 3 business days in advance, the estimate generally must be provided within the applicable federal timeframes: within 1 business day after scheduling for services 3-9 business days away, or within 3 business days after scheduling for services at least 10 business days away. An estimate requested before scheduling is generally due within 3 business days.
The estimate is not a contract and may exclude unexpected care. If a bill from a provider is at least $400 above that provider's estimate, you may qualify for the federal patient-provider dispute process; ordinarily the dispute must begin within 120 calendar days of the initial bill. Keep your estimate and bills. To request an estimate, call 321-877-0007. Learn more at CMS: medical bill rights without insurance.
Privacy and communication choices
See the Privacy Policy, HIPAA notice, and SMS terms. Use the Privia patient portal for personal medical questions and records. Do not submit health information, payment-card details, or identity documents through ordinary website email. You may ask for confidential or alternative communications. Merely using this site or giving us a phone number does not constitute consent to promotional texts.
Acceptable use and intellectual property
Do not attempt unauthorized access, introduce malicious code, disrupt the site, impersonate another person, harvest personal information, or post another person's health information. Content and branding belong to their respective owners. You may view, print, and share links for lawful personal use. Other reproduction or commercial use requires permission unless law allows it. Soriano Med and Privia logos do not imply that this website is a government service or that any outcome is guaranteed.
Outside links, reviews, and availability
Privia scheduling and portal services, Google Maps, and review widgets are operated by third parties under their own terms and privacy practices. Reviews reflect individual experiences; ratings and automated summaries may change and are not clinical evidence or guarantees. We aim for accurate information, but website content may be incomplete, outdated, or temporarily unavailable. Contact the practice to confirm information relevant to your care.
Rights, accessibility, and legal limits
Nothing in these terms limits patient rights, privacy rights, nondiscrimination protections, remedies that cannot lawfully be waived, or responsibility that law does not allow us to exclude. These terms do not impose binding arbitration, waive a jury trial, or authorize disclosure of medical information. Applicable federal and Florida law govern subject to mandatory protections of other applicable law. For accessibility assistance or a concern, see our Accessibility Statement.
Florida patients may request the written statement of patient rights and responsibilities and raise concerns without improper retaliation. See the Florida Patient's Bill of Rights and Responsibilities.

Revisions will be posted with an updated effective date. Separate consents or agreements will be obtained when law requires them.
