We know these documents matter for your work, benefits and daily life. Completing them involves more than a signature: your clinician reviews your medical history, evaluates the requested information and discusses the paperwork with you. Our team also checks completeness, coordinates delivery and keeps a copy in your chart.
The fee reserves clinician and staff time for this work. It applies to provider-completed certifications, not to downloading or filling out routine patient consent forms on this page.
Bring your deadline to our attention when requesting service. Completed originals are collected in person by you or an authorized person.
The American Academy of Family Physicians recognizes charging for administrative work such as FMLA paperwork when contracts and regulations permit. The American Medical Association recommends advance disclosure and fees based on reasonable practice costs. These statements recognize this type of fee; they do not set a standard price or endorse our $150 amount.

In a 2025 study at one academic medical center, 51% of 67 clinician respondents reported completing FMLA paperwork in 15 minutes or less before an electronic-workflow change. This illustrates variation in paperwork time. It does not measure how many practices charge fees or establish an appropriate price.
