HIPAA Notice of Privacy Practices

Your medical information, your rights, and the official Privia notice.

Last updated: November 4, 2026.

Official Privia joint notice

Soriano Med is part of Privia Medical Group. Privia publishes a joint Notice of Privacy Practices for participating providers. Use the complete notice for the full description of permitted uses, disclosures, rights, and complaint procedures.

This local page is a guide to the official notice, not a replacement or a separate authorization.

Your information and your choices

HIPAA provides rights concerning access to health records, requests for correction, confidential communications, restrictions, disclosure accountings, a paper copy of the notice, and complaints without retaliation. Contact the office for help exercising your rights. A legally authorized representative may act on your behalf after authority is verified. Access to available records and routine messages is also provided through the Privia patient portal.

Sensitive records and applicable law

HIPAA and other federal and state laws may apply differently to particular records. The official notice addresses substance use disorder records protected by 42 CFR Part 2, including restrictions on use in proceedings against a patient. Florida law provides additional medical-record confidentiality requirements, including protections for certain HIV, mental-health, and other sensitive information. Requests for disclosure are reviewed under the law applicable to the record.
